Question about Injuries

Anyone know the general rule for the common injury statutes. There seems to be: Probable, Questionable, and Out. Isn't there a doubtful too? Also, most of the questionables say day-to-day. Is there any way to get better information than this?

Also, is there a general percentage that can be attached to these? Is "probably" over 50% the guy will play or is it more like 95%. Same with questionable. All the questionables seemed to say day-to-day next to it.
